Community Renewal is now working with the NHS and local agencies to improve health. Our new project in South West Glasgow is in partnership with the Keep Well programme to encourage people to take up screening and support them in measures to improve their health.

Since April 2008 we have developed a special support initiative for people who require additional support to engage with the Keep Well screening programme. This support consists of a team of Community Health Outreach Workers and Health Case Managers who provide an intensive and holistic support service.

Our outreach workers visit people at home to provide more information on the benefits of attending the health check and also provide practical support in terms of organising suitable appointments and any help needed to get to the appointment. They also provide emotional and practical support for people to attend health improvement services such as Maintaining Wellbeing Sessions, Smoking Cessation, Healthy Eating and Lifestyle, Financial Inclusion and Employability Services.
Our Health Case Managers HCMs work with people who are finding it difficult to engage with a health improvement service or who have been referred to a number of services. The HCMs carry out an holistic assessment with each person and help them to identify their own health priorities and life goals and provide on-going practical support to achieve their goals.

North Lanarkshire

Community Renewal has been a partner in the NHS Lanarkshire Keep Well Project where we provide a community engagement element to the programme to increase the number of people attending screening clinics, as well as a holistic support package to individuals through our dedicated Health Case Manager.

Since our involvement in the Keep Well programme began in August 2007, we have supported approximately 1000 people who required additional help to engage with the Anticipatory Care Programme.

We are now delivering a Peer Group support project for the NHS in Lanrkshire to help people reduce and sustain weight loss

Health Scotland Training

Community Renewal has recently delivered training across Scotland to front line staff in community engagement techniques to help increase the number of people taking up support services within the Keep Well programme
